Community Connector

The Community Connector position is a joint initiative between Whanganui Regional Health Network and Ministry of Social Development. We are seeking a hardworking, passionate person with a commitment to the health and wellbeing of whānau, who has had previous experience working in a community setting.

This is a part-time position (20 hours per week), and you will be working alongside another Community Connector based at the Whanganui Regional Health Network.

 

The purpose of the role is to provide connection and direct support to and with people, so that they can access the right information, the right service at the right time – to support individual and whānau wellness, housing, education, and employment goals. You will be delivering short-term assistance, to prevent and minimise the impacts of hardship.

Existing relationships with a range of health care providers, social services and other external agencies, and the ability to form further collaborative links in the community is crucial, with the aim of creating connected solutions with whānau that are sustainable, build resilience and provide a pathway to independence.
