We have several highly successful programmes to assist with the care of people who have long-term conditions like diabetes, asthma, COPD, heart problems, gout, depression etc.

How We Can Help

 

We have a team of health professionals and kaiawhina who specialise in helping people to manage their conditions with a whānau centered approach. We will work together with you, your whanau, and your health team to develop an integrated care plan.

Your care plan is about the things that matter to you and your whanau. This can mean sometimes we will work with other sectors such as housing, employment, Work & Income NZ, and others to get the support you and your whanau need to improve your health and wellbeing.

We offer free programmes/ workshops that can help you gain skills and knowledge to better manage your long-term conditions at home, these include:

  • Living well with Diabetes Self-Management programme

  • Piki te Ora Self-Management programme

  • Cancer thriving and surviving – coming soon

  • Chronic Pain Self-Management programme – coming soon

  • Building Better Caregivers workshop – coming soon

  • Workplace Self-Management Programme – coming soon

You will learn self-management techniques ‒ from someone who has been on the same journey as you ‒ such as planning, healthy eating and exercising, sleeping, working with healthcare providers, problem solving and decision making to help manage your health condition.

Free Virtual Heart Recovery and Self Care Awareness Programme

What is heart recovery?

Heart recovery is an important process to healing your heart . It is for people who have had a heart event, procedure or diagnosed heart condition.

Cardiac rehabilitation provides you and your family with education, information, physical activity and social support, helping you make essential changes to your lifestyle so you can return to as normal a life as possible.

Free 4 Week Programme

•    Cardiac risk factors
•    Medication

•    Stress management
•    Emotional wellbeing
•    Nutrition
•    Tailored exercise programme

Are You Breathless?
Free Virtual Pulmonary Rehabilitation and Self Care Awareness Programme

Chronic Obstructive pulmonary disease (COPD)


COPD is an umbrella term for a group of respiratory diseases which include emphysema, chronic bronchitis, chronic asthma and pulmonary fibrosis. These conditions make you very breathless and less inclined to exercise or exert yourself. As a result you become unfit and you do even less.


The good news is that even though we are unable to cure your condition, pulmonary rehabilitation classes can improve your fitness and the way you manage your condition.

Free 6 Week Programme

Sessions will include:
• How to manage breathlessness
• How to stay well and avoid flare up's
• How to take your medications
• Manage fatigue and stress
• Eating well with COPD
• Stop smoking support if needed

Our Self-management programmes are free and available in your local community for you and your whanau. The programmes are offered face to face and virtually (coming soon) and are designed to improve wellbeing, especially for those who have long-term medical conditions.

The programme helps you:

  • learn new skills to manage your health condition

  • develop confidence in the way you manage your condition(s)

  • meet others and share similar experience

  • learn new tips for keeping as well as possible

  • feel in control of your life

  • achieve goals and milestones

  • turn obstacles into milestones.

FREE 4 WEEK PROGRAMME
The free Cardiac Rehabilitation and Self Care Awareness Programme is run weekly for four weeks and you can start anytime.

We also provide weekly consults with the programme facilitator, where you have an opportunity to discuss any questions you may have about each week’s topics.

FREE 6 Week Programme
The Pulmonary Rehabilitation and Self Care Awareness Programme is free. It is run weekly for 6 weeks and you can start anytime. Weekly sessions include 2 exercise sessions designed specifically for COPD patients. Followed by education, advice and weekly phone calls.

 

Sessions will include:
• How to manage breathlessness
• How to stay well and avoid flare up's
• How to take your medications
• Manage fatigue and stress
• Eating well with COPD
• Stop smoking support if needed

Don't have any of these?? Don't worry! If you don't have access to the internet, we can still deliver the programme to you at home.

 

Breathing Control

You may be concerned about your ability to exercise because of your breathlessness. We will provide you with strategies to help you manage your breathing during exercise.
