Our team are looking for a  Primary Care Programme Lead to manage and deliver quality work programmes that will support general practice to provide sustainable and equitable health care to our population.

 

The role includes leadership, project management and coordination as well as developmental and maintenance responsibilities for all programmes that have a direct link between WRHN and our general practice teams.

 

The successful candidate will initially be responsible for the delivery of two key programmes of work:

1. Integrated Primary Care Mental Health Programmes

2. Healthcare Home Model of Care

 

You will be required to provide effective leadership, communication and coordination across Network of Providers to support sustainable models of care, increase access and equity to a range of health services and support general practice teams to deliver a holistic view of wellbeing (based on Te Whare Tapa Wha).
