Empowering Whānau Through Collaborative Care Coordination

We are seeking a highly motivated Registered Nurse Care Coordinator to join our team at Whanganui Accident and Medical in collaboration with Whanganui Regional Health Network (WRHN) and who will be pivotal in driving a changing model that recognises the benefits of high-quality interdisciplinary team based care that is committed to Whānau Ora (wellbeing).
About the role:

Identify individuals and whānau with complex needs who require care coordination across multiple providers and settings.

Identify individuals and whānau currently unenrolled with a primary care provider and support a pathway to enrolment.

Facilitate smooth transitions of care, ensuring timely information exchange and clear accountability.

Collaborate with whānau to establish goals and assess health and wellbeing needs.

Build strong relationships with whānau, healthcare professionals, Iwi, hauora māori providers and social service providers, forming a comprehensive "circle of care."

Develop and maintain care planning with people in collaboration with whānau and healthcare teams.

Provide tailored support and education to empower whānau in self-management of their health.

Monitor needs, progress towards goals, and identify areas requiring intervention.

Connect whānau to available community resources and referrals.

Lead and support interdisciplinary team collaboration to deliver effective care.
We are looking for someone with:

A relevant qualification in health (e.g., nursing.)

Experience in care coordination or a related field.

A strong understanding of the health and social care sectors.

Excellent communication and interpersonal skills to build relationships with whānau and healthcare professionals.

A collaborative and team-oriented approach.

Commitment to whānau-centred care and cultural safety principles.

Proficiency in Te Tiriti o Waitangi principles.
