Providing this information helps us deliver safe and effective healthcare. If you choose not to provide information that is relevant to your care, it may limit the services we are able to provide.

 

We use your personal information to provide your healthcare, coordinate services, undertake clinical audit, quality improvement, service planning, and meet our legal and funding obligations.

 

We may share relevant information with other healthcare providers involved in your care, or where authorised or required by law. We will not share your information with anyone else without your consent unless authorised or required by law.

 

You have the right to request access to your personal information and to request correction of any information you believe is inaccurate.

If you are not satisfied with our response, or you prefer to do so, you have the right to make a complaint to the Office of the Privacy Commissioner or the Health and Disability Commissioner.
