Registered Nurse Prescriber

Registered Nurse Prescriber (or Senior Registered RN on Prescriber Pathway)
Focus: Diabetes and other Long Term Conditions / 0.8 – 1.0 FTE / Permanent
Gonville Health is seeking a Registered Nurse Prescriber – or a Senior Registered Nurse on the pathway to becoming a Nurse Prescriber – with a passion for diabetes and other long-term condition management to join our friendly and supportive team.
Our practice serves a diverse, low socio-economic population, and we are proud to provide care that is high quality, equitable, and culturally safe. This role is ideal for someone who enjoys building strong relationships with patients and whānau, takes a proactive approach to managing health, and values working as part of a collaborative primary care team.
In this role, you will:

Lead proactive diabetes care for our patients, as well as seeing a range of general and acute presentations within your scope of practice.

Work closely with GPs, Nurse Practitioners, Pharmacist, Health Coach and the wider interdisciplinary team to provide integrated, patient-centred care.

Support patients through education, medication management, and self-management strategies.

Use proactive recall and data systems to ensure continuity of care and improved outcomes.

Contribute to service development and quality improvement across the practice.
We are looking for someone who:

Is a Registered Nurse Prescriber or a Senior Registered Nurse working towards Nurse Prescriber status, with an interest in diabetes management and other long term conditions

Communicates effectively and builds strong therapeutic relationships.

Is proactive, organised, and adaptable

Culturally responsive and committed to the principles of Te Tiriti o Waitangi

Works well in a collaborative, multidisciplinary environment.
