Care Coordinator – CPCT - Te Waipuna (1.0 FTE)

 

Empowering Whānau Through Collaborative Care Coordination

We are seeking a highly motivated Care Coordinator to join our innovative team in the Whanganui Locality. This is a pivotal role within the National CPCT Strategy, working to build a high-quality healthcare system focused on whānau ora (wellbeing).

 

About the role:

  • Identify individuals and whānau with complex needs who require care coordination across multiple providers and settings.

  • Facilitate smooth transitions of care, ensuring timely information exchange and clear accountability.

  • Collaborate with whānau to establish goals and assess health and wellbeing needs.

  • Build strong relationships with whānau, healthcare professionals, Iwi, and social service providers, forming a comprehensive "circle of care."

  • Develop and maintain Proactive Plans of Care (PoCs) in collaboration with whānau and healthcare teams.

  • Provide tailored support and education to empower whānau in self-management of their health.

  • Monitor needs, progress towards goals, and identify areas requiring intervention.

  • Connect whānau to available community resources and referrals.

  • Lead and support interdisciplinary team collaboration to deliver effective care plans.

 

We are looking for someone with:

  • A relevant qualification in health or social services (e.g., nursing, social work).

  • Experience in care coordination or a related field.

  • A strong understanding of the health and social care sectors.

  • Excellent communication and interpersonal skills to build relationships with whānau and healthcare professionals.

  • A collaborative and team-oriented approach.

  • Commitment to whānau-centred care and cultural safety principles.

  • Proficiency in Te Tiriti o Waitangi principles.
